A Clinicial Review of Peptic Ulcer Perforation of Stomach and Duodenum



Abstract
This clinical review was made of 30 cases in which perforated gastric and duodenal peptic ulcers were
treated surgically at the Se-Jong General Hospital from October 1983 to September 1985.
1. Duodenal ulcer perforation was the most common incidence involving 18 cases (60%), while the remaining cases included 8 cases (27%) of gastric ulcer perforation, 1 case (3.3%) of gastric perforation due to carcinoma, and 3 rases (10%) of traumatic gastric perforation.
2. The sex distribution assumed a ratio of fourteen to one with 28 males and 2 females. The age range in the -majority of the patient was in the second decade of life (43%); the youngest was 19 and the oldest was 74 of age.
3. In 3 cases (10%), perforated peptic ulcers were combined with hemorrhage.
4. Prior to operation, peptic ulcer symptoms were noted in many cases (93%), and the predominant period was less than 1 year period.
5. Duodenal ulcer perforations were 2 times more common than gastric ulcer perforation. The most common perforated sites was lesser curvature side of body in gastric ulcer and the first portion of duodenum in duodenal ulcer.
6. As the emergency surgical procedures, Billroth II subtotal gastrectomy was performed in 11 cases (36.7%),
vagotomy with pyloroplasty 9 cases (30%), primary closure 7 cases (23.3%), vagotomy with antrectomy 1
cases (3.3%), vagotomy with hemigastrectomy 1 case (3.3%), and 1 case (3.3%) of total gastrectomy.
7. Postoperative complications were 7 cases (23%). Wound infection in 2 cases (29%) was the predominant feature.
8. Overall mortality rate was 3.3%.
